Referral Process

We appreciate the faith you place in us by referring your patients to us. We recommend that all patients with suspected sleep apnea be evaluated by a sleep medicine provider prior to pursuing treatment. When we proceed, it is our policy to keep you updated on the patient’s progress and refer them back to you for a follow-up sleep study with their oral appliance to confirm the outcome.

1. Diagnosis completed and confirmed by sleep medicine provider (SMP)

2. Submit referral to our clinic along with patients evaluation notes and sleep study results (online form)
